    Tara O’Connell is a Home Infusion Dietitian with Advocate Home Health Services in Downers Grove, IL, where she coordinates comprehensive nutrition support for patients across the upper Illinois region. She collaborates with interdisciplinary teams to manage clinical care and billing processes while providing oversight for enteral and parenteral therapies. Tara’s commitment to patient care and safety is central to her practice, ensuring evidence-based interventions and proactive monitoring for optimal outcomes. Her experience in both acute care and long-term acute care hospitals has given her valuable insight into optimizing discharge planning and creating safer transitions for patients moving from hospital to home. Drawing on this expertise, she strives to bridge gaps in continuity of care and enhance the patient experience through education, collaboration, and process improvement.
